Please read these guidelines carefully before completing the following Passport Application Form. -

Jamaican Passport Application Requirements
Renewal of Passport/Damaged Passport (Adults):
-
Completed passport application form
-
Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
-
Expired Passport
-
Original Birth Certificate
-
Original Marriage Certificate, where applicable
-
Original Death Certificate, where applicable
-
Original Adoption Register, where applicable
-
Divorce Judgment, where applicable
If born abroad, ,instead of the Birth Certificate, please submit one of the following:

Certificate of Naturalization
-
Letter of Certificate of Citizenship
-
Certificate of Registration
Renewal of Passport/Damaged Passport (Minors):

First-Time Applicant (Adults):
All first time applicants must visit the Consulate General to be interviewed by a Consular Officer.

Driver’s License from country of residence

Other identification issued by a Federal/State/City authority, which has an expiration date (subject to approval by a Consular Officer)

Mailed Applications:
Section F and one (1) photograph must be certified by a Notary Public. Their stamp should be affixed in the area provided. These persons need not be Jamaicans or Commonwealth Citizens.
NB: Photocopies of documents, even if notarized, will not be accepted. All original documents will be kept and returned at the end of Passport processing period.

Total includes US$30.00 Consular and FEDEX second business day shipping & handling fees. Please make sure your address is clearly written, and apartment numbers are included where necessary.
The Consulate General of Jamaica accepts only Money Orders and Cash (Do not send cash in the mail). Personal checks and credit cards are not accepted. The processing time for Passports is six (6) weeks.
Jamaicans wishing to renew their passports can obtain application forms by clicking on the Passport Forms Link above, print, complete and send to the nearest consulate in your area or the Embassy in Washington, DC.
Application forms can also be obtained by contacting the Embassy of Jamaica in Washington D.C., the Consulate General of Jamaica in New York or other Jamaican Consulates near to them. The form is free.

NOTE: While we provide application forms, we do not process passports at the Consulate in Seattle. This has to be done at either the Embassy of Jamaica in Washington, D.C., or at one of the Consulate General (New York or Miami).
